Name Change Procedure in Telangana is three stages and First stage, to start with getting of Police Verification, before changing of old name into new name you ought not included in any criminal offenses thus a Police Verification Certificate is really require.
Second stage is Name change procedure for the citizen who are residing in Telangana, intends to change his/her name, shall apply, in the first instance, in the form in Annexure-I to the Tahsildar/Executive Magistrate having the respective jurisdiction for issue of an official communication in the form of a letter or memorandum to the effect that he is a Citizen of India.
The Tahsildar or the Executive Magistrate of respective jurisdiction, may on the basis of information available in the application furnished by the applicant and after such inquiry as he thinks fit, issue a letter or memorandum in the form of Annexure-II to this Order, to the effect that on the basis of the information available on record, he is qualified for recognition as a Citizen of India. The Tahsildar or Executive Magistrate concerned jurisdiction shall exercise due caution and satisfy himself about the applicant’s claim to Indian Citizenship before issuing the letter or memorandum.
Third stage is Publication of Annexure-II endorsement in the Telangana Gazette. Subsequently, the applicant shall, thereafter, submit an application to the Commissioner of Printing, Hyderabad along with a covering letter and Annexure-II Certificate issued by the Tahsildar/Executive Magistrate concerned to publish under private advertisements in the Gazette, on payment of the publication charges.

PROCESS TO CHANGE NAME:
Name Change process has been divided in to three categories, viz...
1 Name Change for Majors those who
attained the age of 18 years (can apply directly)
2
Name Change for Minors should apply through his/her parents.
3
Name Change process for the employees of State Government.
For the first initial two stages an Affidavit in the minimum value of stamp paper by giving relevant information such as your name, your father/husband name, date of birth and age along with your present address by including your reason to change of your old name into new name and authorize it. In the event of minors their respective parents should give affidavit on behalf of their minor daughter or son.
In respect of minors -- The police Verification Certificate is not essential in most of the case and in certain age limit as the police authorities may deems fit.
In respect of the Employees of State Government -- may apply to Gazette Publication by giving an affidavit incorporating his/her service details in the minimum value of stamp paper and duly confirming from their superior authorities. The police clearance certificate and certificate of revenue officials is need not necessary.
For all categories Gazette publication charges are prescribed by Telangana Government in time to time.
After getting Gazette Publication
copy the process of name change is finished.
The persons who have attained the
age of 18 years can apply directly. In case of minors, their parents may apply
by producing Identity, proof of Citizenship viz., Ration Card, Pan Card,
Passport etc. and for State Government Employees no need of apply for
Tahsildar/District Magistrate/Addl. Dist. Magistrate.
